Introduction Local anaesthesia may be employed in hernia operations, either on its own or combined with general anaesthesia. The choice of technique will be influenced not only by local resources and skills, but also by patient preference. Advantages of Local Anaesthesia for Hernia Repair With a careful technique, local anaesthesia causes minimal physiological disturbance. This may be particularly useful for patients with cardiovascular or respiratory disease for whom there may be advantages in avoiding a general anaesthetic. The absence of postoperative sedation or drowsiness allows early ambulation and diminishes the requirement for recovery facilities. Local anaesthesia provides postoperative analgesia for up to four hours and may be administered by the surgeon. When adrenaline is mixed with the local anaesthetic (normally in a dilution of 1:200,000) useful vasoconstriction is produced resulting in a relatively bloodless field.
Disadvantages of Local Anaesthesia for Hernia Repair Surgery on the awake patient under local anaesthesia must be carried out gently. Although pain sensation is usually blocked by the anaesthetic, traction on certain tissues, particularly the peritoneum, is uncomfortable. The patient should be warned that some sensation may be experienced during the operation but that it will not be painful. Larger hernias, particularly those with incarcerated bowel may prove unsuitable for local anaesthesia.

Most people believe that anesthesiologists are the doctors who administer medications which keep them from feeling pain and sensations. However, few people realize that beyond ensuring the patient's comfort, today's anesthesiologists' primary role in the operating room is to make informed medical judgments to protect and regulate the patient's critical life functions that are affected by the surgery being performed. Also, these medical specialists are the doctors who will immediately diagnose and treat any medical problems that might arise during surgery or the recovery period. Anesthesiologists need a wide range of knowledge about medications, internal medicine, how the human body works, and its responses to the stress of surgery. As physicians, anesthesiologists are responsible for administering anesthesia to relieve pain and for managing vital life functions, including breathing, heart rhythm, blood pressure, and brain and kidney functions during surgery.

Introduction
One of the most important tools available to veterinarians to thoroughly and painlessly treat pets is the advent of modern day anesthetics. These anesthetic agents allow us to sedate and anesthetize animals with negligible chance of serious side effects. The lack of significant complications from anesthesia is due to a combination of expertise, thorough pre-anesthetic testing, and state of the art anesthetic and monitoring equipment. We are equipped to anesthetize any pet from a finch that weighs 15 grams (it takes 454 grams to make up one pound), to pets that weigh several hundred pounds. We are also particularly proficient in anesthetizing older ( geriatric ) pets and pets with medical problems. The goals of anesthesia are to minimize anxiety and eliminate pain. In addition, from the surgical point of view, anesthesia allows profound muscle relaxation. This is helpful in every surgery because the procedure will go quicker and incisions can be made smaller. In certain types of surgery like fracture repair , this muscle relaxation is crucial for success.

Parents When Your Child Has Surgery
Breathing through a mask that covers her small face, the little girl slips into a state of unconsciousness. Her mother and father release their hold on her tiny hands, then reach out for one another's. They watch as the anesthesiologist wheels their daughter out of the induction room, then down the hall to surgery. They worry about their child's safety and comfort. Before the use of modern anesthetics, surgery was rarely attempted - or hastily performed. Thanks to modern pediatric anesthesia, however, this little girl will not feel pain during her operation. And as she recovers, what pain she does have can be effectively controlled. Anesthetics, used in surgery as far back as ancient times, inhibit communication between the brain and the nervous system and thereby create insensibility to pain. Now modern-day anesthetics make it possible to do the surgeries your child needs - virtually no matter how long they take or how complicated they are. Richard Helffrich, MD, a pediatric anesthesiologist, describes anesthesia associated with surgery in three stages: the preoperative workup, care during surgery (intraoperative care), and postoperative care, which includes recovery and pain control after the surgical procedure.
