HIV attacks the immune system, resulting in a chronic, progressive illness and leaving infected people vulnerable to opportunistic infections and cancers. The median time from infection to AIDS diagnosis now exceeds 10 years. AIDS is fatal. There is no cure.
HIV is transmitted through:
  • unprotected sexual intercourse needle-sharing pregnancy, delivery and through breast feeding (from an infected mother to her infant) occupational exposure in the health care setting
History
The first case of AIDS in Canada was reported in 1982. Up to December 31, 2003, 19,344 AIDS cases have been reported to Health Canada but the total number of AIDS cases that have occurred in Canada since the epidemic began is estimated to be close to 20,000 (after adjusting for under-reporting and reporting delay). The HIV/AIDS epidemic is actually several epidemics, occurring in specific populations. The early epidemic primarily affected men who have sex with men (MSM) and those who received blood and blood products. The current epidemic, measured from 1997, primarily affects injection drug users (IDU), MSM and, increasingly, women and Aboriginal people. Although these epidemics spread at different rates, the social factors and conditions that lead to the spread are similar.

1. I just found out I am HIV-positive, but I don't have insurance. Can I get insurance now? Life and disability insurance will be much more difficult. In some states, you can purchase small amounts on a "guaranteed issue" basis from special risk brokers. Most people, however, will only be able to get life and disability coverage by going to work full time as part of a large employee group. 2. I do not know if I am HIV-positive, but I have had several sexually-transmitted diseases and I have used drugs. Will this affect my application for insurance? If you are applying for health insurance in an "open enrollment" environment, you will have the same opportunities as anyone else (see answer to question 1). In states that do not offer that opportunity, or if you are applying for life and disability insurance, a history of sexually transmitted diseases might lead the insurer to decline your application or at least to request a blood test, including an HIV antibody test, and a physician's statement. A history of drug use may cause the insurer to decline your application or to charge you a higher premium. Rules will vary from company to company. 3. If I apply for insurance, may I be required to submit to an HIV test?

When was HIV discovered and how is it diagnosed?
How is HIV spread (transmitted)?

HIV is present in the blood and genital secretions of virtually all infected individuals, regardless of whether or not they have symptoms. The spread of HIV can occur when these secretions come in contact with tissues such as those lining the vagina, anal area, mouth, or eyes (the mucosal membranes), or with a break in the skin, such as from a cut or puncture by a needle. The most common ways in which HIV is spreading throughout the world include sexual contact, needle sharing, and transmission from infected mothers to their newborns during pregnancy, labor (the delivery process), or breast- feeding. (See the section below on treatment during pregnancy for a discussion on reducing the risk of transmission to the newborn.) The spread of HIV by exposure to infected blood usually results from sharing needles, as in those used for illicit drugs. HIV also can be spread by sharing needles for anabolic steroids taken to increase muscle, tattooing, and body piercing. To prevent the spread of HIV, as well as other diseases including hepatitis, needles should never be shared. At the beginning of the HIV epidemic, many individuals acquired HIV infection from blood transfusions or blood products, such as those used for hemophiliacs. BURUNDI: IRIN interview with HIV/AIDS Minister Sindabizera
NAIROBI (IRIN) - The Burundi government says although the HIV infection prevalence in urban areas has stabilised at 18.6 percent, it is concerned by a significant increase in the level of infection in rural areas. At 1 percent in 1989, HIV prevalence in rural areas was 7.5 percent by 2001, the Burundi minister for HIV/AIDS issues, Genevieve Sindabizera, told IRIN on Wednesday. She attributed the stabilisation in urban areas to a higher literacy rate, better means of communication, and increased acceptance and availability of condoms. In order to avoid further deterioration of the situation in rural areas, she said a national action plan targets vulnerable groups by way of an information campaign carried out through peer educators, the promotion of condom use, counselling of people living with HIV/AIDS, the reduction of the epidemic's socioeconomic impact on people by the promotion of revenue-generating activities, and the social integration and education of orphans.
With an average annual per capita income of US $140, the economic impact of the HIV/AIDS pandemic has been dramatic in a country wracked by years of civil strife. Burundi's HIV/AIDS secretariat has estimated that if transmission of the virus continues at the current rate, life expectancy will drop below 40 years in 2010, compared to 60 years if there were no AIDS.

Until the mid-1990s, most of the countries of the former Soviet Union appeared to have been spared the worst of the HIV epidemic. Mass screening of blood samples from people whose behaviour put them at risk of infection showed extremely low levels of HIV, right up to 1994. At that time the whole of Eastern Europe put together had around 30,000 infections among its 450 million people, Western Europe had over 15 times as many cases, while in sub-Saharan Africa over 400 times as many people were living with the virus. The AIDS epidemic in Eastern Europe and Central Asia now shows no signs of declining. Some 230,000 people were infected with HIV in 2003, bringing the total number of people living with the virus to 1.5 million. AIDS claimed 30,000 lives in the past year. Worst affected are the Russian Federation, Ukraine, and the Baltic states (Estonia, Latvia, and Lithuania), but HIV continues to spread in Belarus, Moldova and Kazakhstan, while more recent epidemics are now evident in Kyrgyzstan and Uzbekistan. It is now estimated that around 1 million people aged 15-49 are living with HIV in the Russian Federation (although various estimates from that country put the figure at between 600,000and 1.5 million). Driving these epidemics is widespread risky behaviour- injecting drug use and unsafe sex- among young people. Extraordinarily large numbers of young people regularly or irregularly engage in injecting drug use. This is reflected in increasing HIV prevalence among injecting drug users throughout the former Soviet Union.
