What is Agoraphobia?

Agoraphobia is the fear of going out into public places. Agoraphbia can occur with or without panic attacks Individuals with severe agoraphobia should usually start both medication and therapy as soon as possible. Without the medication, such an individual might not be able to make full use of the therapeutic process. People with mild to moderate symptoms might chose a combination approach or therapy alone. Homework between situations, and coaching from family members or therapists help one gradually face the feared situations. *vignettes are fictional examples

Treatment is essential One of my trickiest moments as a medical student was in my psychiatry finals when I was introduced to an apparently healthy young woman. I was told that she had a large collection of traffic tickets for abandoning her car in a one-way street at the entrance to a small bridge which crossed a river which ran through the centre of town. The examiner invited me to ask a few relevant questions to help establish the diagnosis. Ask a few questions Fortunately after a few nerve racking seconds I was able to put the pieces of the jigsaw together. I asked the following questions:
  • did she have a fear of being in some sort of situation where there was no obvious or immediate escape? how would she feel strolling on a long beach or in the crowd at a football match?

Most people think that it means a fear of open spaces but that's not quite right. Agoraphobia is an anxiety about being in places or situations from which it might be hard to escape or where you might embarrass yourself, lose control or have a panic attack Fears might include:

    - Phillip W. Long, M.D. (Internet Mental Health) The word "agoraphobia" originally meant "fear of the open marketplace" and meant fear of open spaces. Now it's usually used as a term to describe the fear of going into unfamiliar territory. Agoraphobics have had panic attacks before and are afraid of having a panic attack away from home or away from someone they feel safe with. Agoraphobics might describe their symptoms as panic, panic attacks, anxiety, nervousness, fear, spells, rapid heartbeat, hyperventilation, fear of death, fear of going crazy or the fear of fear itself. Agoraphobics experience anxiety about being in places or situations from which escape might be difficult. They are afraid panic-like symptoms might occur. Agoraphobic fears could include being outside the home; being in a crowd; being on a bridge; and traveling. Agoraphobia usually causes people to avoid these situations or have someone accompany them - sometimes causing serious interruptions and inconveniences and often leading to feelings of low self-worth. Agoraphobics often have a lifestyle which is severely limited by their condition.

    Meg Logan has not been farther than two miles from home in six years. She has agoraphobia, a debilitating anxiety disorder that entraps its sufferers in the fear of leaving safe havens such as home. Paradoxically, while at this safe haven, agoraphobics spend much of their time ruminating over past panic experiences and imagining similar hypothetical situations. In doing so, they create a narrative that both describes their experience and locks them into it. Constructing Panic offers an unprecedented analysis of one patient's experience of agoraphobia. In this novel interdisciplinary collaboration between a clinical psychologist and a linguist, the authors probe Meg's stories for constructions of emotions, actions, and events. They illustrate how Meg uses grammar and narrative structure to create and recreate emotional experiences that maintain her agoraphobic identity. In this work Capps and Ochs propose a startling new view of agoraphobia as a communicative disorder.

    I received a phone call last night from a lady with Agoraphobia ("Marie") whom I had never met. I drove 20 miles to her VERY rural home this morning to see if I could help. Nice lady. She answered the door with her toddler grandson ("Johnny") at her side.
    Marie had agoraphobia for at least 25 years. She can go outside her home but rarely goes beyond 50 yards without feeling intense body heat, panic and other fear responses. She refers to it as "my illness." The first order of business is always establishing rapport. We had done a fairly good job on the phone the night before and, since I knew she had great concerns for Johnny, I immediately turned my attention toward him. I got down on one knee (at his physical level) and, in a form of child talk (his style of language), asked him how old he was. He warmed up right away and informed me that he was practicing to be a bell. I said "ding-dong" as he moved his head back and forth. Rapport is easy if we are flexible and genuinely care. Marie loved it. That's important. It establishes trust and opens one up to these belief-violating procedures. The subtle energies that circulate throughout the body have been largely ignored (until recently) by western scientists. As a result, our use of them for emotional and spiritual healing has been sparse at best. With EFT, however, we consider these subtle energies to be the front running cause of emotional upsets. As a result, we generate results that are FAR beyond those of conventional methods. Even a cursory review of the case histories on this web site will reveal that.
