Only after many months of official investigation is a final accident investigation report published. That report may contain hundreds of pages of highly technical information. Generally, each issue of Accident Prevention Accident Prevention fills the gap between nonspecialist reporting, which can be superficial (and inaccurate), and official accident investigation reports, which typically are lengthy, highly technical and not readily available to the public. Accident Prevention is important reading. Monthly. Established 1948. Two-color newsletter, 8-inch x 10-inch format, usually four to 16 pages. Subscription included with FSF membership. Others: US$240/year. Back issues in print are available at $15 for members/$40 for nonmembers each, plus shipping. Prices do not include shipping. Payment must be made in advance. U.K. Air Accidents Investigation Branch For consummate skill and leadership in reconciling the competing requirements of accident investigators, aircraft operators, manufacturers and aviation regulators through his work on international committees to enhance flight-recording systems and their value in aircraft-accident investigation. As head of the Flight Recorder Section of the U.K. Air Accidents Investigation Branch (AAIB) from 1987 to 2003, Mr. Sheppard earned global respect for his contributions to working groups of the European Organization for Civil Aviation Equipment, which develop and update performance specifications for cockpit voice recorders and digital flight data recorders. He has worked to develop future recorder-performance specifications for communications navigation and surveillance/air traffic management (CNS/ATM) and video images. He chaired and/or represented AAIB on flight-recorder working groups of the International Civil Aviation Organization, Joint Aviation Authorities and International Society of Air Safety Investigators. Mr. Sheppard also has helped the U.K. Civil Aviation Authority, U.K. aircraft operators and developers of flight recorders to identify acceptable solutions for compliance with technical specifications and operational requirements.

NASA announces Columbia accident investigation board

NASA Administrator Sean O'Keefe today announced the members of the Space Shuttle Mishap Interagency Investigation Board, which will provide an independent review of the events and activities that led up to the tragic loss of the seven astronauts Saturday on board the Space Shuttle Columbia. The board's first meeting is scheduled for tomorrow at Barksdale Air Force Base in Louisiana. Retired U.S. Navy Admiral Harold W. Gehman, Jr., who co- chaired the independent commission that investigated the attack on the U.S.S. Cole in Aden, Yemen, Oct. 12, 2000, and once served as the commander-in-chief of U.S. Joint Forces Command, will chair the panel. "While the NASA family and the entire world mourn the loss of our colleagues, we have a responsibility to quickly move forward with an external assessment to determine exactly what happened and why," said Administrator O'Keefe. "We're honored to have such a distinguished panel of experts, led by Admiral Gehman."

When a military aircraft crashes, there is always at least one accident investigation; if it resulted in death or injury, there is generally more than one investigation, each conducted by the military. At times, a third may be conducted by the NTSB and/or the FAA. It is the usual case that two independent investigations are considered. The first is a formal Aircraft Accident Review Board, the composition of which is determined by regulations of the service involved. Since its purpose is limited to the promotion of aviation safety and often because the equipment and aircraft involved are complex and of a confidential nature, the military customarily refuses to make any disclosure regarding the composition of the Board, the factual findings made, or any report of its findings, opinions and conclusions. A second informal Collateral Board investigation is also prepared. In the Navy, this is made pursuant to the Judge Advocate General Manual and is referred to as the "JAG Report".

The NTSB aviation accident database contains information from 1962 and later about civil aviation accidents and selected incidents within the United States, its territories and possessions, and in international waters. Generally, a preliminary report is available online within a few days of an accident. Factual information is added when available, and when the investigation is completed, the preliminary report is replaced with a final description of the accident and its probable cause. Full narrative descriptions may not be available for dates before 1993, cases under revision, or where NTSB did not have primary investigative responsibility. The following information is available:
  • Database Query - interactive search capability for the NTSB database, updated daily; see the general instructions before using the form for the first time. Monthly lists - accidents sorted by date, updated daily. Completed investigations - list of cases scheduled for release of probable cause in the next 2 weeks. Downloadable datasets - one complete dataset for each year beginning from 1982, updated monthly in Microsoft Access 95 MDB format; this FTP site also provides weekly "change" updates and complete documentation.
