Scholz's Prostate Cancer Research Institute has an exceptionally informative web site concentrating in innovative approaches to prostate cancer. [ See Box The Prostate Gland below. ] As with any cancer, if it is advanced or left untreated in early stages, it can eventually spread through the blood and lymph fluid to other organs. Fortunately, prostate cancer tends to be slow growing compared to other cancers. As many as 90% of all prostate cancers remain dormant and clinically unimportant for decades. This high incidence of latent or incidental malignancy is unique to the prostate gland. Most older men eventually develop at least microscopic evidence of prostate cancer, but it often grows so slowly that, as one specialist has written, many men with prostate cancer "die with it, rather than from it."

The serum prostate-specific antigen (PSA) assay is the test most commonly recommended for the purpose of screening. However, when PSA screening is examined critically from the standpoint of the principles of screening, evidence from prospective studies to support the routine use of PSA testing is lacking. Data suggest that screening often detects what may be indolent, nonaggressive prostate cancer. The treatment of such a cancer with radiation or radical prostatectomy can result in significant morbidity, including urinary incontinence and impotence, without a proven decrease in mortality. Evidence from randomized clinical trials in support of routine PSA screening is urgently needed. T he purpose of screening tests is to identify disease in asymptomatic persons at a stage when treatment will alter the natural history of the condition. There is now a widespread call to incorporate routine prostate-specific antigen (PSA) screening for prostate cancer into the care of men over age 50. However, little prospective evidence shows that the benefits of such testing outweigh the risks. The ideal way to assess the efficacy of screening is with prospective randomized clinical trials; unfortunately, no such trial of prostate cancer screening has been conducted.

The findings may mean that men should be cautious about a new kind of treatment called testosterone replacement therapy, being tested in older men who see a decline in general health and vigor. A study of more than 750 men showed those with the highest levels of free testosterone in the blood were the most likely to have prostate cancer. "Since testosterone replacement therapy increases the amount of free testosterone in the blood, older men considering or receiving testosterone replacement should be counseled as to the association until data from long-term clinical trials becomes available," J. Kellogg Parsons, a urologist at Johns Hopkins University who led the study, said in a statement. The association between free testosterone and prostate cancer risk in older men was not affected by height, weight, percent of body fat or muscle mass, Parsons told a meeting of the American Urological Association in San Francisco.
