Extractions: Volume 3, Issue 4, Winter 1998 EDITOR'S NOTE Over the past twenty years, teen pregnancy has received increased attention, demonstrated by policy reform, program development, and services for young teen mothers and their children. In this issue, Suzanne Bartholomae and I review the consequences associated with teen pregnancy and the risk factors that could serve as intervention targets. We argue that as practitioners we have the opportunity via intervention to test the adolescent pregnancy models put forth by basic researchers. One such program, "Project Taking Charge," is shared by Jerelyn Schulz. Aspects of the successful development and implementation of this national program are discussed. In addition, Judy Kimberly, Jerry Bean, and Bob Hughes examine the importance and methods of evaluating teen pregnancy prevention programs. Extractions: Violence Against Women Violence against women is a public health concern of considerable importance in developed and developing countries. Womens greatest risk of violence comes from men they know, often husbands or male family members. In addition to violating the human rights of women, such violence poses significant risks to womens health, including their reproductive health. These risks include immediate physical and psychological injury, as well as less obvious risks such as gynecological disorders, unsafe abortions, pregnancy complications, unwanted pregnancy, and sexually transmitted infectionss, including HIV. Domestic violence, the most prevalent form of violence against women, remains a relatively obscured and ignored issue in reproductive health research and programming. In countries like South Africa, with a high prevalence of HIV, extremely high rates of rape and other forms of violence against women have been documented and may play an important role in womens greater vulnerability to HIV infection. Violence against women may be a factor in many domains of the Councils areas of researchincluding reproductive tract infections, HIV/AIDS, and family planning.

Extractions: Advanced Search Preterm labor is the leading cause of perinatal morbidity and mortality in the United States. It is characterized by cervical effacement and/or dilatation and increased uterine irritability before 37 weeks of gestation. Women with a history of preterm labor are at greatest risk. Strategies for reducing the incidence of preterm labor and delivery have focused on educating both physicians and patients about the risks for preterm labor and methods of detecting preterm cervical dilatation. Methods used to predict preterm labor include weekly cervical assessment, transvaginal ultrasonography, detection of fetal fibronectin and home uterine activity monitoring. As yet, it is unclear if any of these strategies should be routinely employed. At present, management of preterm labor may include the use of tocolytic agents, corticosteroids and antibiotics. P reterm labor is the leading cause of perinatal morbidity and mortality in the United States. It usually results in preterm birth, a complication that affects 8 to 10 percent of births in the United States each year.

Summary: Researches wanted to determine whether the association between OC use and risk of breast cancer is influenced by family history of the disease. It was a historical cohort study of 426 families of breast cancer probands diagnosed between 1944 and 1952. Participants were a total of 394 sisters and daughters of the probands, 3002 granddaughters and nieces, and 2754 women who married into the families. Researchers concluded that women who have ever use earlier formulations of OCs and who also have a first-degree relative with breast cancer may be at particularly high risk for breast cancer. However, more studies are needed of women with a strong family history who have used more recent lower-dosage formulations of OCs are needed to determine how women with a familial predisposition to breast cancer should be advised regarding OC use today. Comment: Don¹t sound the alarm yet. No one has ever said that OCs protect against breast cancer, but there is also no evidence that OCs cause breast cancer in much larger studies. This study could be subject to several types of bias and needs confirmation. If the ³older-higer² dose OCs had significant increase, we should have seen it before now. As the author admits, the same may not hold for presently used OCs.

Extractions: A growing number of women across America are having their first babies at age 40 or even older. Overall, first births among women over 30 rose to a record 22% of all births in 1995, as opposed to 5% of births in 1975. According to the National Center for Health Statistics, the birth rate for women aged 40-45 years rose 20% between 1990 and 1995, and increased 74% during 1981-95. The rising birth rate along with the increasing number of women in this age group means that there were more babies born in 1995 to mothers in their 40s than in any year since 1966. What explains this pattern of delayed childbearing? According to a study by Susan Coady, Human Development and Family Science faculty member at The Ohio State University, there are several reasons for waiting to start a family. Women who wait usually have more financial security, more emotional maturity, and more time to commit to their marriages and to their careers before focusing their energy on children. It isn't so much that people aren't interested in having children, but they are just busy with their lives. Many couples have waited until they've established careers, bought a house and fixed it up, and then realized that it's getting late and they're running out of time.

Extractions: ABSTRACT Adolescent pregnancy, the disproportionate number of births to unmarried adolescents, the potential disadvantages for both mothers and their children, and the commensurate costs to society have received the attention of researchers in a variety of disciplines. This article reviews and synthesizes the disparate literature on psychosocial factors associated with adolescent pregnancy using Bronfenbrenner's ecological model. Social influences within the macrosystem, mesosystem, and microsystem are examined. Policy and service delivery recommendations are offered. Adapted from the physical sciences to explain human behavior, "Bronfenbrenner's (1979) ecological model of human development... conceptualizes ecological space as operating on different levels of systems, each of which is incorporated within the next" (Franklin, 1988, p. 340). At the most basic interactional level is the microsystem, which is the pattern of activities, roles, and interpersonal relations experienced by the individual in a given setting. The mesosystem involves interactions among settings. The next level, the exosystem, includes settings that affect the individual but with which the individual does not interact directly (exosystem variables are not covered here, since there are so many other variables that have a more direct impact on adolescents). The macrosystem includes the cultural variables that influence the individual.
