Extractions: The Senior Citizens' Legal Services program in Winona and Albert Lea offers free legal advice, representation and community education to people age 60 and over living in the 11 counties served by the Southeastern Minnesota Area Agency on Aging. For the seniors program there are not the same income limits as there are for the representation of non-seniors. The Winona office serves the following counties: Dodge, Fillmore, Goodhue, Houston, Olmsted, Wabasha and Winona. The Albert Lea office serves seniors living in Freeborn, Mower, Rice and Steele counties. There are approximately 76,900 senior citizens living in our service area. Of these, nearly 9,000 live in poverty and almost 5,000 reside in nursing homes. Many of these senior citizens experience problems with regard to government benefits, Medicare, Medicaid, Social Security, housing and consumer rights. Our primary service goal is to provide high quality civil legal services through individual case representation and limited advice and referral. Our services are prioritized to try to ensure that those in greatest social and economic need receive them. We also conduct legal education events throughout our service area with the objective of increasing the awareness among senior citizens of their rights so that they can more effectively seek and obtain the assistance they may require. We provide legal education information, either in written form or oral presentations, to other agencies or programs working with the elderly. We do not charge a speaking fee for our presentations.

Extractions: Introduction MSHO integrates Medicare and Medicaid financing and benefits and acute and long-term care services in a market-based managed care delivery system under a Medicare+Choice (M+C) contract between the federal government and the State of Minnesota. The State of Minnesota contracts with health plans to provide and manage health care and support services for seniors age 65+ in the seven-county metro area and in three rural counties in Minnesota. MSHO provides individualized care coordination to meet the chronic care needs of seniors. In September 2001, the state expanded the MSHO model to enroll people from 18 through 64 with physical disabilities in a program called Minnesota Disability Health Options (MnDHO). Nationally, MSHO is the first and largest state-sponsored demonstration that integrates Medicaid and Medicare for people in all settings. As a model that shows promise of meeting the chronic care needs of aging baby boomers, MSHO has attracted the interest of other states, providers, researchers, national organizations such as the American Association for Retired People and National Governors Association, and even other countries. Development of MSHO has been supported by grants from the Robert Wood Johnson Foundation. Minnesota participates in the RWJF Medicare and Medicaid Integration Project (MMIP) a 10-state initiative focused on improvement of chronic care and administered by the Center on Aging, University of Maryland.

In addition, HealthPartners has expanded its travel benefit from $5,000 to $100,000 per calendar year. The travel benefit allows members to receive services from any Medicare provider while traveling or temporarily residing outside of Minnesota for up to six months. HealthPartners is also making additional improvements to the services it offers to seniors. These changes include improving access to transitional care beds for members who need nursing facility care between a hospital stay and going home. HealthPartners will also provide more help to members in accessing services and information about clinics, nursing homes and hospitals. "While more work needs to be done, the 106th Congress took important steps to save Medicare + Choice from imminent collapse, due to insufficient funding," said Mary K. Brainerd, HealthPartners' executive vice president and chief operating officer. "The additional funding authorized by Congress helped reduce the red ink situation that was forcing health plans to drop out of the program and therefore disrupt the benefits of millions of seniors, including thousands of seniors within Minnesota. Cost estimates for 2001 could mean even larger cost of care increases. That means the money already approved by Congress lags far behind the cost of providing Medicare + Choice benefits in our current market.

Experience Works is a national, non-profit organization that provides training and employment services for people over 55 years with low incomes. "It's an excellent program," commented Marion Snare of Buffalo, who is a field operations assistant for Experience Works in Wright County. What does it do? The program matches employers with seniors for jobs up to 22 hours per week, said Luella Prehn, field operations assistant for the organization in McLeod County. In fact, for non-profit organizations such as nursing homes, the program is great because the program pays for the seniors' wages, Prehn said. For example, the wages of a senior employed at a medical facility would be paid for by the program, Prehn said.
