The family eagerly accepted assistance from Partnership for Healthier Kids, which provided the necessary referrals. The little girl got the treatment she needed for her broken foot and returned to school. Program Description - Inova Health System's Partnership for Healthier Kids is an intensive prevention and care coordination program focused on schools, families and neighborhoods. The Partnership has spent two years developing and testing programs to bring health and safety assessment, preventive health care education, and care coordination to children and their families through elementary schools. The program has been built on thorough and sound collection of data to pinpoint needs and opportunities. The Partnership was created to increase the percentage of children who receive immunizations, injury and smoking prevention services, and have a source of coordinated primary care for their preventive and episodic healthcare.
