Dear Reader, Endometriosis occurs when some of the tissue which lines the uterus (the endometrium) begins to grow in another part of the body. In most cases, this growth develops in the pelvic area on the ovaries, the lining of the pelvic cavity, ligaments, or the fallopian tubes. Because these growths are made of endometrial tissue, they usually behave like the endometrium, responding to the hormones of the menstrual cycle. Each month, they build up tissue and slough it off. As a result, internal bleeding, inflammation, cysts, and scar tissue can develop in the affected areas. Common symptoms of endometriosis include: extreme pelvic pain during menstruation, ovulation, and/or sexual intercourse; excessive menstrual flow; fatigue; lower back pain; infertility; or, no symptoms at all. Endometriosis can also cause other serious problems, such as ruptured ovarian cysts and an increased risk of ectopic (tubal) pregnancy. The disease can be debilitating, even interfering with normal daily activities, for days, weeks, and even months at a time. Fortunately for you and your girlfriend, diagnosis and treatment will make a difference.

Although they may suffer significant symptoms ranging from pelvic pain to infertility, most of these women do not know that they have endometriosis,. Physicians' understanding of (1) the clinical presentation of endometriosis, (2) its proper diagnosis and staging, and (3) the management of its sequele have improved dramatically over the past few years. The result has been better, more cost-effective patient care. Definition Endometriosis is the presence of endometrial tissue (normally found only on the inside of the uterus) in locations outside the uterus. This tissue reacts to estrogen and progesterone. The usual location is in the pelvis (on the ovaries, fallopian tubes, uterus, or bladder), but endometriosis has also been found in sites outside the pelvis (including omentum, small intestine, appendix, anterior abdominal wall, surgical scars, diaphragm, lung, urinary tract, and musculoskeletal and neural systems). This endometrial tissue reacts to hormonal changes during the menstrual cycle, just as endometrial tissue lining the inside of the uterus reacts during the normal ovulatory cycle. Prevalence and Incidence The prevalence and incidence of endometriosis depends on the population of women being studied, ranging from 1 to 50%. It has been reported to occur in 10 - 15% of women undergoing diagnostic laparoscopy, 2 - 5% of women undergoing tubal sterilization, 30 -40% of infertile women having laparoscopy, and 14 - 53% of women with pelvic pain.

Most of these implants are on the left pelvic sidewall, under the ovary. The white, black, and deep scarred implants should be excised to assure complete removal. White Endometriotic Lesions The outlined area contains both black endometrial implants (left side) and white scarred implants (right side). The entire area is hormonally active and should be excised. This area overlies the right side of the pelvis beneath the right ovary.

What is endometriosis? Why is it so painful and how does it affect a woman's ability to have a child? These answers are not simple ones, but to women who suffer from endometriosis and it's complications these answers are needed so desperately, it pains the heart as well. According to Dr. David Michael, faculty specialist in Obstetrics and Gynecology at Mt. Sinai Hospital in Chicago, Ill., endometriosis is "the presence of functioning endometrial glands (the glands that normally line the uterus which produce tissue that prepare the uterus for conception) that occurs outside the uterine cavity. These glands can be located anywhere inside the abdominal cavity or attached to the organs within." The areas inside the abdominal cavity that can be affected by endometriosis include the abdominal wall, kidneys, ovaries, bladder, fallopian tubes, the outside of the uterus itself or even the areas in-between these organs, which are referred to as "cul-de-sacs."

The patient's sister is four years younger than the patient and is herself infertile. Physical examination is unremarkable with the exception of the pelvic and rectal examinations. Bimanual pelvic examination reveals mild tenderness in the left adnexal area with apparent fixation of the left ovary to the uterus. Vaginal ultrasonography revealed a tender 3 cm left ovarian cyst. Rectal examination was painful with marked induration approximately 3 cms from the anus. In a minority of cases, this can be caused by endometriosis, a condition in which small pieces of the endometrium (womb lining) grow outside the womb. As I began to gather research I thought to myself how quick and easy this was going to be. Endometrial growths may also be found in the bladder, bowel, cervix, and less commonly in places like the lungs or arms. Symptoms Diagnosis Diagnosing endometriosis, or "endo" as we refer to it, can be extremely difficult. The symptoms listed above can also be signs of a variety of other conditions from ovarian cysts to a pelvic infection. Endo was once falsely called the "career woman’s disease." We now know that it was titled this because working women generally had more resources to seek continuous medical care.

It was described in medical literature more than 300 years ago and has since been recognized as a chronic, painful, and often progressive disease in women. The causes of endometriosis are unknown. Endometrial Implants Endometriosis. Endometriosis occurs when cells from the mucus membrane lining the uterus ( endometrium ) form implants that attach, grow, and function outside the uterus, generally in the pelvic region. [ See Box The Female Reproductive System.] Endometrial implants consist of both following cell types: Gland cells. These cells secrete hormones and other fluids and are normally located in the uterine lining. Stroma cells. These are the framework cells that build supportive tissue. Endometrial cells contain receptors that bind to estrogen and progesterone, which promote uterine growth and thickening. During endometriosis these cells become implanted in organs and structures outside the uterus, where these hormonal activities continue to occur, causing bleeding and scarring.

All other manifestations of endometriosis exhibit a wide spectrum of expression.The lesions are usually fond on the peritoneal surfaces of the reproductive organs and adjacent structures of the pelvis,but they can occur any where in the body.The size of the individual lesions variesfrom microscopic to large invasive masses that erode into underlying organs and cause extensive adhesion formation.In the uterineendometriosis the lesion is in the miometrium which is invaded by endometrium from within,this condition is generally called adenomiosis. The cause of endometriosis is unknown.Several theories have been offered to explain its occurrence,but none have sactisfactorily explained all of the features of the disease. Atheory of retrograde menstruation was proposed during the 1920s. It was postulated that endometrioses occurred because viable fragments of endometrium were shed at the time of menstruation and passed through the uterine tubes,the tissuebecame implanted on peritoneal surfaces and grew into endometriotic lesions.This theory is simple and atractive but it does not explain why all women do not develop endometriosis or the rare cases of endometriosis in the lung ,brain,or other soft tissues or in nonmentruating subjects (S. Turneror absent uteri).
