For the companion avian practitioner, larger psittacine species represent patients that are suitable for corrective measures. Miniaturized techniques might be used in smaller patients. THE STENT: DESCRIPTION AND TECHNIQUE The stent used is machined from solid virgin Teflon by McAllister Technical Services at West 280 Prairie Avenue, Coeur d'Alene, ID 83814, tel: 1-208-772-9527. The unit is 12.7 or 16 mm in diameter with eight pre-drilled holes for suture placement in the subcutaneous flange. The central stem is 1.5 mm deep and 4.5 mm in diameter with a 1.5 mm orifice. The outer rim is 8 mm in diameter, allowing the skin to tuck under the edge and thereby preventing the healing dermis from closing over the implant. The stents are inert and non-reactive. One post-surgical problem has been occasional plugging of the stent with mucous or congealed tissue fluid which is easily removed with a needle. This has occurred a few times post-operatively in some cases, but this problem eventually clears. Post-operative antibiotics have not been used and infected surgical sites have not occurred. To prevent post-surgical plugging in larger psittacines, it might be useful to use a larger stent with a 3 mm orifice and a 16 mm diameter external flange.

For infants with acute/serious respiratory symptoms, segmentectomy or lobectomy is the treatment of choice. The incidence of complications is low. Asymptomatic patients may not require any definitive treatment. Most respiratory symptoms do not progress and resolve within a year of age.

Extractions: Emphysema - an Introduction E mphysema is a devastating disease caused by destruction of lung tissue. The usual cause is cigarette smoking. As tissue is destroyed in the lung, large airspaces or holes appear in the lung. The elastic properties of the lung are decreased and the airways narrow. This results in overinflation of the lung (lungs too large), large decreases in the patient's ability to move air in and out of the lungs, and decreases in the ability of oxygen to pass into the blood from the lungs. Emphysema, when severe, is as devastating and debilitating as cancer, severe heart failure or any other severe chronic disease. The most troublesome symptom is severe shortness of breath of even the most trivial effort. Patients may develop a whole host of secondary symptoms such as anxiety and depression, severe pulmonary infections, heart failure, and respiratory failure. Mortality is considerably increased in emphysema patients. Traditional Treatment T raditional medical treatment consists of smoking cessation (the first step), medications to open the airways of the lungs as much as possible (bronchodilators), medications to decrease inflammation in the airways (steroids), oxygen, and antibiotics for pulmonary infections. Inhalers for the administration of bronchodilators and steroids are frequently used.

Mice that inhale cigarette smoke and develop emphysema not only show the enlarged alveoli and destruction similar to human emphysema, but they also have deposits of tar and chronic inflammation, as do patients with emphysema. Therefore, we feel that this model is the most appropriate for testing potential therapeutic drugs. We're enthusiastic about our current research to devise a treatment based on the inhalation of retinoids. LRRI scientists already have the needed expertise in aerosol drug delivery. Because inhaled retinoids directly target lung tissues, lower doses of retinoids may be effective when administered in this manner. We hope that inhalation of retinoids will avoid many of the problems of systemic toxicity.
