People with classical autism show three types of symptoms: impaired social interaction, problems with verbal and nonverbal communication and imagination, and unusual or severely limited activities and interests. Symptoms of autism usually appear during the first three years of childhood and continue throughout life. Although there is no cure, appropriate management may foster relatively normal development and reduce undesirable behaviors. People with autism have a normal life expectancy. Autism affects an estimated two to 10 of every 10,000 people, depending on the diagnostic criteria used. Most estimates that include people with similar disorders are two to three times greater. Autism strikes males about four times as often as females, and has been found throughout the world in people of all racial and social backgrounds. Autism varies a great deal in severity. The most severe cases are marked by extremely repetitive, unusual, self-injurious, and aggressive behavior. This behavior may persist over time and prove very difficult to change, posing a tremendous challenge to those who must live with, treat, and teach these individuals. The mildest forms of autism resemble a personality disorder associated with a perceived learning disability.

Recent studies indicate that as many as 1 in 500 children could have autism. At HHS, increased activities are underway to better understand autism, its causes and potential treatments. Research funding for autism has nearly quadrupled in the last five years - from $10.5 million in fiscal year 1995 to $40 million in fiscal year 1999. The estimate for NIH autism funding in fiscal year 2000 is $45.5 million. Furthermore, the Centers for Disease Control and Prevention (CDC) is currently heading studies to determine autism prevalence rates in selected areas of the U.S., and also funds autism research at Marshall University Autism Training Center in West Virginia. In the absence of a cure, interventions designed to alleviate specific symptoms are available. The best studied therapies include educational, behavioral and pharmacological interventions. Children who receive these therapies show progress in social and language skills.

Extractions: University of Iowa Autism is a severe developmental disorder. The actual cause is not known. A child with autism does not develop a normal personality or interactive skills. Impairment is seen in communication skills, social interaction, and emotional development. Autism usually can be detected before the child is 30 months old. It is an uncommon but lifelong disorder that affects about 4 children out of 10,000. Boys are likely to have it 3 or 4 times more often than girls. Autistic children exhibit odd characteristics in 4 primary areas: SOCIAL RELATIONS: Infants may have a delayed or absent social smile. They may fail to interact with caregivers. They may not reach out to be picked up and held. Pleasure is not shown in cuddling or other contact. A lack of social interest is present as the child ages. BEHAVIOR: Repetitive behavior occurs in the child's second year. Examples are flicking a light switch on and off, passing a toy back and forth from one hand to the other, or walking around a room feeling the walls. Common body movements might include hand flicking or twisting, spinning, or head-banging. Stopping these behaviors or disturbing their surroundings may cause the child to respond with a temper tantrum.

T here has been considerable debate recently over whether there has been an increase in the levels of autism in children under 16, and whether there is an underlying upward trend. 2. To answer these questions obviously requires an accurate monitoring system in each Health Authority area, together with joint arrangements with education and social services, to produce a register of cases. This is in addition to the application of consistent diagnostic criteria, and other considerations. 3. In 1998, the then Junior Health Minister for England, Paul Boateng, stated in response to a Parliamentary Written Question by Helen Southworth MP, that: ...........commissioners are advised to have arrangements in place to use both local and national information to build up a picture of the health needs of people............with autism in their locality. We also encourage health authorities to work together closely with local authorities in profiling both the volume, and needs, of............people with autism (Parliamentary Written Answer of 9th June 1998) 4. To verify what the actual situation was, in the light of the then Ministers comments, at Health Authority/Board level, every Health Authorities in England and Wales, the Health Boards of Scotland, the Education and Library Boards of Northern Ireland and also the Social Services Departments of London were approached. Each body was asked to give a brief indication of how it assessed the incidence of child autism in its area.

Extractions: Background Recently changed guidelines for child health surveillance in the United Kingdom (U.K.) suggest targeted checks only, instead of the previously conducted routine or universal screening at 2 years and 3.5 years. There are concerns that these changes could lead to a delay in the detection of children with autism and other pervasive developmental disorders (PDD). Recent U.K. studies have suggested that the prevalence of PDD is much higher than previously estimated. This study establishes to which extent the routine checks contributed to the early detection and assessment of cases of PDD. Simultaneously we have evaluated the process involved and estimate the prevalence of PDD in our district.
